Our Team would find it incredibly useful to have the ability to ask for additional donor information (multiple custom questions) on the simple and branded widgets. The ability to make certain fields optional or required before the donation would be a bonus.

We are seeking volunteers to trial our new custom questions model in exchange for feedback about it. This highly requested enhancement to the second-generation payment form will allow nonprofits to add custom questions to the donation checkout process, save custom questions in a Custom Question Library for reuse with other fundraising pages and export a Question Report CSV of custom questions and donors’ responses. Hello! Please note that our custom question fields are not fully released to all Give Lively accounts yet, but I've enabled it for you in your Give Lively portal. If you navigate into your core profile or campaign section, you will see a new tab called "Additional Questions" that will allow you to collect mailing addresses and data from one custom field that you can use for phone number or company information.
